To qualify for your rebate, purchase any of the
PocketDISH products listed above between 5/15/05 and 6/30/06. And , if you're not already, become a DISH Network subscriber in the same time period. Finally, follow these steps to receive your rebate:
1. Download and fill out the
PD Rebate Form completely
2. Circle the qualifying purchase on your dated sales receipt or invoice
4. Include the original UPC code from the
PocketDISH packaging.
5. Include a copy of your current bill with DISH Network Services (October 2005 or newer).
